Capital Funding
Capital funding is split between two funding streams, the Housing with Care Fund (HCF) which focuses on accommodation and immediate care and the Health and Social Care Integration and Rebalancing Capital Fund (IRCF) which focuses on the creation of Integrated Health and Social Care Hubs, investing in premises and facilities for Adult and Children’s care.

Housing with Care Fund (HCF)
The Housing with Care Fund (HCF) is a Welsh Government initiative designed to improve housing options for people with care and support needs. This capital programme promotes independent living, enabling individuals to thrive in their communities while receiving the care they need. By focusing on innovative housing solutions, the HCF helps people maintain their independence, reduces hospital admissions, and ensures care is delivered closer to home. The fund also provides critical support for unpaid carers and vulnerable groups, including older adults, children and young people with complex needs, and individuals with learning disabilities or mental health challenges.
Administered through Regional Partnership Boards (RPBs), the HCF targets investment into three main areas: housing with care, intermediate care facilities, and smaller-scale projects such as community-led initiatives and home adaptations. This fund aligns with the Welsh Government’s broader vision for sustainable, inclusive communities and works in tandem with other programmes like “A Healthier Wales” to deliver meaningful outcomes. The HCF reflects the government’s commitment to fostering independence, enhancing care quality, and supporting Wales’ transition to a more integrated health and social care system.
Health and Social Care Integration and Rebalancing Capital Fund (IRCF)
The Health and Social Care Integration and Rebalancing Capital Fund (IRCF) supports the transformation of health and social care services in Wales by investing in integrated health and social care hubs and rebalancing residential care markets. This innovative programme focuses on three key priorities: creating co-located hubs to provide seamless community-based care, expanding not-for-profit adult residential care, and eliminating private profit from children’s residential care. These initiatives aim to deliver more equitable, community-focused services that improve well-being across Wales.
Led by Regional Partnership Boards (RPBs), the IRCF promotes collaboration between health boards, local authorities, and third-sector organisations to deliver integrated services. Projects must align with Welsh Government priorities, such as decarbonisation and town-centre-first principles, ensuring long-term sustainability. By enabling strategic investments, the IRCF helps create community hubs that serve as one-stop destinations for a range of services, empowering individuals to access tailored care and support where and when they need it most.

Dementia Hwb
The Swansea Dementia Hwb is operated by Dementia Friendly Swansea and acts as a one-stop-shop for information on dementia, supporting carers, people who have or are seeking a diagnosis of dementia, professionals and members of the public wanting to educate themselves on the topic.
It is based in the Quadrant Shopping Centre in Swansea conveniently placed near the doors to the bus station, capital funding has been used to help adapt the existing unit to better suit the Hwb, allowing for private consultation areas and quiet spaces.
