This priority focuses on the development of new models of financially sustainable and integrated community health and care to support people to live safely within their own homes and communities.

There is a particular emphasis on:

• Home from hospital services.
• Reducing unscheduled care admissions for people over 65.
• Reducing the time spent in an acute hospital setting following an unscheduled care admission for people over 65.
• Reducing unnecessary recourse to long term care for people over 65.

The Communities and Older People’s Programme delivers against the Transforming Health and Care Services at Home priority.
